1. Agreement to Terms

By accessing and using the Consultation Form ("the Platform"), you agree to be bound by these Terms of Service. If you do not agree to these terms, you must not use the Platform.

2. Description of Service

The Platform provides a multi-tenant consultation form system that enables medical clinics, pharmacies, and healthcare providers ("Clinics") to create and manage branded consultation forms for their patients. The Platform handles the collection, storage, and management of patient consultation data.

3. Clinic Responsibilities

As a Clinic using the Platform, you agree to:

  • Provide accurate and complete information when registering your clinic account
  • Maintain the security of your account credentials
  • Comply with all applicable data protection laws, including GDPR
  • Obtain appropriate patient consent before collecting consultation data
  • Use the Platform only for lawful purposes
  • Not share your account credentials with unauthorised persons
  • Maintain an active subscription to continue using the Platform

4. Subscription and Payment

Subscription terms:

  • Subscriptions are billed monthly or annually in advance
  • Payment is processed securely through Stripe
  • Subscriptions automatically renew unless cancelled
  • You may cancel your subscription at any time through your billing settings
  • Cancellation takes effect at the end of the current billing period
  • No refunds are provided for partial billing periods
  • We reserve the right to change pricing with 30 days' notice

5. Data Protection and Privacy

The Platform is designed to protect patient data. However, as a Clinic, you are responsible for:

  • Ensuring you have appropriate legal basis for processing patient data
  • Complying with all applicable data protection regulations
  • Implementing appropriate security measures for your account
  • Notifying patients about how their data is collected and used

6. Intellectual Property

The Platform, including its design, functionality, and underlying code, is the property of the Platform owners. You retain ownership of all data you submit to the Platform, including patient consultation data and clinic branding materials.

7. Limitation of Liability

The Platform is provided "as is" without warranties of any kind. We do not guarantee uninterrupted or error-free operation. To the maximum extent permitted by law, we exclude all liability for any loss or damage arising from your use of the Platform.

8. Termination

We reserve the right to suspend or terminate your account if you breach these Terms of Service. Upon termination, you will lose access to your account and data. You may export your data before termination.

9. Changes to Terms

We may update these Terms of Service from time to time. Material changes will be communicated via email or through the Platform. Continued use of the Platform after changes constitutes acceptance of the updated terms.
